Upper Eyelid Lift Procedure Steps

Anaesthesia

You will be seated comfortably in an adjustable surgical chair. A local anaesthetic is used to numb the eyelid area, and mild sedation can be offered to help you feel more at ease.

Incision

With your eyes closed and your head supported, Mr Yapa makes precise markings along the natural eyelid crease to guide the incision, keeping any scarring discreet. The incision is then shaped according to the amount of skin and fat to be adjusted.

Adjustment and Refinement

Mr Yapa carefully removes excess skin and, where needed, any small pads of fat beneath it.

Closing the Incisions

The incisions are closed with fine sutures placed to support healing and keep scarring to a minimum.

Risks and Complications

Bruising and Swelling

Usually temporary and typically settling within 1 to 2 weeks.

Scarring

Incisions are made as discreetly as possible, and any resulting scars are usually faint and sit within the natural crease of the eyelid.

Dry Eyes

Some patients notice dryness afterwards. This is generally temporary and can be managed with eye drops or medication.

Difficulty Closing the Eyes

Closing the eyes can feel unusual for the first few days. This normally resolves as healing progresses.

Upper Eyelid Lift Recovery

0 – 48 Hours

Straight after surgery it is common to notice temporary symptoms such as a burning sensation, sensitivity to light, watery or itchy eyes, and mild discomfort. Cold compresses and over-the-counter pain relief help to ease this. It is best not to drive while your vision may be affected, so please arrange for someone to collect you.

1 – 2 Weeks

Some swelling and bruising around the eye is normal and will gradually settle over this period. Many patients feel ready to return to work after two to three days, though some bruising may still be visible, and sunglasses can shield your eyes while discreetly covering any early signs.

2 – 6 Weeks

The remaining swelling continues to subside and the eye begins to look and feel more like itself, with the result becoming clearer as the area settles.

3 – 6 Months

The fine scars that sit within the eyelid crease may take a few months to fade completely. Keeping the area shielded from the sun during this period helps them settle well.

Who is a good candidate for an upper eyelid lift?

Good candidates are usually those bothered by heavy or hooded upper lids, whether due to hereditary factors or the natural loosening of the skin over time. For some people, this can begin to affect their vision, making surgery worthwhile on functional grounds as well. Others choose the procedure to create a more defined crease, sometimes known as a double eyelid. Beyond this, ideal candidates are in good general health, have healthy eyes, and hold realistic expectations. Mr Yapa will confirm whether the procedure is suitable for you at your consultation.

What is the difference between an upper and lower eyelid lift?

Blepharoplasty is the umbrella term for eyelid surgery, and is divided into two procedures. An upper eyelid lift addresses excess skin and fatty tissue on the upper lid, while a lower blepharoplasty, or Lower Eyelid Lift, targets concerns beneath the eye, such as lower eye bags, puffiness and fine lines. Both can be carried out safely and effectively by an experienced surgeon, and Mr Yapa will help you decide which is right for you.

What surgical techniques are used for an upper eyelid lift?

There are two main approaches, and the right one is decided at your consultation:

  • Where the eyelid droops more significantly, a ptosis correction may be needed to tighten the muscle that lifts the lid. In most cases, this means carefully tightening the levator aponeurosis, the tissue connecting the lifting muscle to the eyelid. If that muscle is very weak, a small strip of tissue from the forehead can be used as a sling, borrowing lifting support from the brow.
  • The standard incisional technique uses an incision within the natural crease of the upper lid, designed to remove excess skin, adjust fatty tissue and tighten muscle, while keeping any scarring discreet.

What is the best age for an upper eyelid lift?

There is no single right age for cosmetic surgery of this kind, as everyone’s needs are different. Many patients notice heaviness in the upper lid from around their forties onwards, but some choose the procedure earlier if genetic factors are causing upper lid heaviness to affect their vision. We always recommend a consultation to confirm whether the procedure is right for you and to discuss every option.

How should I prepare for lower eyelid surgery?

As a general guide: stop smoking for at least 2 weeks beforehand to support healing; disclose all medications, as some may need to be paused; arrange transport and support at home for the first day or two while your vision settles; and above all, follow your surgeon’s guidance, which forms your comprehensive plan for a smooth recovery.

Can I combine an upper eyelid lift with other procedures?

An Upper Eyelid Lift is often combined with a Brow Lift to address heaviness and drooping in the upper eye area. It can also be paired with a lower eyelid lift for a fuller refresh of the eye area, or with a facelift where jowls and midface changes are also a concern. Mr Yapa will help you decide on the right combination during your consultation.

Why is a brow lift often combined with an upper eyelid lift?

The skin above the eye doesn’t sit in isolation; it hangs from the brow. If the brow has descended or become heavy with time, some of what looks like excess eyelid skin is in fact brow skin that has migrated downwards. Two people can have identical amounts of true eyelid skin excess and look quite different, purely because of where their brows sit.

This matters for a reason that isn’t always obvious. When the upper lids feel heavy, most people compensate without realising it by lifting the brows using the forehead muscle. It becomes a constant, low-level reflex; it makes the eyes feel lighter and the field of vision more open. The brow position you see in the mirror is often not the brow’s resting position; it’s a held position.

Once heavy eyelid skin is removed, the reason for that reflex disappears, and the forehead relaxes. The brow then settles to its true resting height, which may be lower than the one you were used to seeing. The result can be a return of upper lid hooding within months of an otherwise well-executed blepharoplasty, not because too little skin was taken, but because the brow was never addressed.

For this reason, brow position is carefully assessed at the consultation, including where the brow sits when the forehead is fully relaxed. In selected patients, resetting the brow at the same time as eyelid surgery yields a more stable, longer-lasting result than eyelid surgery alone and avoids the need for a second procedure later. It is not required for everyone; many patients have good brow support and need only the eyelid addressed.
