What is the SMAS, and why does a facelift need to address it?

Beneath the skin of the face lies a continuous, elastic sheet of fibrous and muscular tissue known as the SMAS. This is the superficial musculoaponeurotic system. It connects the facial muscles to the overlying skin and holds the soft tissues of the face in position. With age, this layer loses its elasticity and descends, carrying the cheek fat and skin downward. That descent produces jowling along the jawline, loss of cheek definition and deepening folds that run from the nose to the mouth.

The skin follows the SMAS. It is not the cause of the change, which is why tightening the skin alone produces a pulled look that does not last. It is the SMAS that must be released and repositioned, and that is precisely what a deep-plane facelift does.

The deep plane technique divides the retaining ligaments that tether the SMAS to the deeper facial structures. Once those attachments are released, the SMAS, the facial fat and the overlying skin can be lifted and repositioned as a single unit in the direction the face has genuinely descended. Because the layers move together, no layer is stretched against another, and the face is restored to a position it once held rather than pulled into a new one.

The Preservation Deep Plane Approach

Deep Plane Facelift Recovery

0 – 48 Hours

You will wake from surgery with a bandage around the face to protect the incision sites. An overnight stay is typically recommended. Swelling and bruising are normal. Sleeping with your head elevated will help them settle more quickly. Cold compresses and rest are beneficial in these early stages.

1 – 2 Weeks

A compression garment may be worn for the first two weeks to manage swelling and support healing. Avoid lifting anything heavy and rest wherever possible. Most patients feel comfortable returning to light activity by the end of week two.

3 – 4 Weeks

Swelling will have reduced considerably by this point. The rejuvenating effect is visible as soon as the dressings come off, and becomes clearer as swelling continues to settle.
